SMART METER HEALTH EFFECTS SURVEY

Even if you have already posted a testimonial on the web, please fill out our survey questionnaire. Answers to our survey's specific questions are needed to gain an accurate assessment of overall health impact. If you do not experience health effects from smart meters please do not fill out this survey, because it is about health effects only and is not a prevalence survey.

We hope to present the results of this survey as testimony in the ongoing Maine Public Utilities Commission (PUC) investigation into the safety of smart meters. This is a precedent-setting case because the burden rests on the PUC to guarantee safety. The PUC deadline for testimony is now January 25, 2013, so please submit your completed survey before January 15 to allow time for analysis. The results may also be useful in future proceedings in Maine and elsewhere. We welcome survey submissions from all over the world.

Our goal is to investigate the relationship between smart meters and health effects. The survey results will provide a database for research that can be used for legal proceedings, the media and scientific publications.

The survey will take about 15 minutes to complete.
Before clicking on the link above to take our survey, you may want to have on hand the the following information:

1) Roughly what is and/or was the total number of smart meters within about a 50 foot radius around your residence, including those (if any) on your residence?
2) How many of each type (Electric, Gas, Water) of smart meter(s) were/are on your residence (or on your building and relatively close to you; and
3) Optional: for the Electric smart meter: AMI or AMR type, Brand/Manufacturer, and if you have it, the FCC ID number from the face or side of the meter under one of the bar codes; it usually begins with "FCC ID". (The FCC ID number identifies brand and model and transmitter info and is not unique to your particular meter.)

OPTIONAL ADDITIONAL INFORMATION ABOUT OUR SURVEY:

This survey will not attempt to reconstruct a complete testimonial through its questions, nor to obtain details of all experiences with smart meters. Our purpose is to extract in a uniform way, particular types of information which will be useful in a database to be used for research, reporting and testifying on the health safety of smart meters. No studies have yet been done to see if smart meters have any biological effects. This survey will be a first step in determining if there is a relationship between smart meters and health effects.

Our questions have mostly multiple-choice answers, some consisting of selection of number ranges. This actually makes it easier for you to make answer entries, and very importantly enables automated tabulation and analysis of the data, which would be much more difficult if text or numerals were freely entered. If there are parts of your story which the multiple-choice answers supplied do not give you the opportunity to tell, you may use the Comments Section at the end of the survey for this purpose. If this is the case, you may wish to take notes as you go along on things to comment on at the end, because this is a forward only survey - you cannot go back once you have gone on to the next page.

If your health or attention span is such that you need 2 or 3 separate sessions to complete this questionnaire, you can do that. If you need to take a break, simply pause for up to an hour. If you pause for longer than an hour, you can then continue by refreshing your browser. If you need to shut down your computer to take a longer break, first click the text at the bottom of your screen that says: "Save and continue later", which will be present in the border on the bottom of your screen on pages 2, 3 and 4 of this survey.

After you submit your survey online you will be sent an email with a PDF of the survey questions with your answers, and a link to view the summary report of all surveys received. You will also be given the opportunity to print out a hard copy of a blank survey to give or mail to people who have been affected by smart meters and cannot use a computer. If you will not be taking the survey yourself and know someone who has been affected by smart meters and cannot use a computer, you or they can call me (see contact info just below) and I will arrange to get them a hard copy they can fill out by hand.

Your participation is extremely important, as the larger the number of completed surveys submitted, the more useful the results will be. After you submit your survey, you will be given the option to attach an affidavit if you wish, which will strengthen the impact of your answers.

SECURITY:
This survey is being conducted via SurveyGizmo. SurveyGizmo has personal information protection certifications from both Safe Harbor (a US Dept. of Commerce program for compliance with European Union privacy laws) and HIPAA (a US federal law that establishes standards for the privacy and security of health information). Your personally identifiable information (last name, street address, email and phone number) will remain in a secure SurveyGizmo computer database, accessible only to the author of this survey via a specially encrypted password. All survjeys and data reside on SurveyGizmo's servers, which use Advanced Encryption Standard (AES) 256 bit encryption, the method used by banks and by the US National Security Agency for classified top secret information. There are multiple layers of proactive security, and data are never exported, sold, rented or used in any way by SurveyGizmo. Neither SurveyGizmo nor the author of this survey will share any of your personally identifiable information with anyone without your express written permission.
